Pharmacology

Ponatinib is a kinase inhibitor. Ponatinib inhibited the in vitro tyrosine kinase activity of ABL and T315I mutant ABL with IC 50 concentrations of 0.4 nM and 2.0 nM, respectively. Ponatinib /Iclusig inhibited the in vitro activity of additional kinases,

PDGFR, FGFR, EPH receptors and SRC families of kinases, and KIT, RET, TIE2, and FLT3. Ponatinib inhibited the in vitro viability of cells expressing native or mutant BCR-ABL

including T315I. In mice, treatment with ponatinib reduced the size of tumors expressing native or T315I mutant BCR-ABL when compared to controls.

Dosage & Administration

Recommended dose: 45 mg taken orally once daily with or without food

Hepatic Impairment: 30 mg orally once daily. Modify or interrupt dosing for hematologic and non-hematologic toxicity

Side Effects

Headache, dizziness, constipation, or loss of appetite may occur. If any of these effects persist or worsen, tell your doctor or pharmacist immediately.

Tell your doctor right away if you have any serious side effects, including: weight gain, numb/tingling skin, pain/numbness/burning feeling in fingers/toes, fainting, fast/slow/irregular/pounding heartbeat, eye pain/swelling/irritation, slow wound healing, sudden severe headache.

Ponatinib /Iclusig sometimes causes side effects due to the rapid destruction of cancer cells (tumor lysis syndrome). To lower your risk, your doctor may add a medication and tell you to drink plenty of fluids.

Tell your doctor right away if you have symptoms such as: low back/side pain (flank pain), signs of kidney problems (such as painful urination, pink/bloody urine, change in the amount of urine), muscle spasms/weakness.

Get medical help right away if you have any very serious side effects, including unusual bleeding/bruising, black/bloody stools, vomit that contains blood or looks like coffee grounds, shortness of breath, seizures.

This medication may raise your blood pressure. Check your blood pressure regularly and tell your doctor if the results are high. Your doctor may control your blood pressure with medication.

This medication may lower your ability to fight infections. This may make you more likely to get a serious infection or make any infection you have worse. Tell your doctor right away if have any signs of infection (such as fever, chills, persistent sore throat, cough).

For women of childbearing age, ponatinib /Iclusig may affect your ability to have children. Ask your doctor for more details.

A very serious allergic reaction to this drug is rare. However, get medical help right away if you notice any symptoms of a serious allergic reaction, including rash, itching/swelling (especially of the face/tongue/throat), severe dizziness, trouble breathing.

Pregnancy & Lactation

Based on its mechanism of action and findings in animals, Ponatinib /Iclusig can cause fetal harm when administered to a pregnant woman. There are no available data on Ponatinib use in pregnant women.

In animal reproduction studies, oral administration of ponatinib /Iclusig to pregnant rats during organogenesis caused adverse developmental effects at doses lower than human exposures at the recommended human dose. Advise pregnant women of the potential risk to a fetus.

There is no data on the presence of ponatinib /Iclusig in human milk, the effects on the breastfed infant or on milk production.

Use in Special Populations

Pediatric Use: Safety and effectiveness have not been established in pediatric patients.
